Each social worker caries a caseload. Their responsibilities include assessing each participant, developing and providing an individual plan of care that meet each participants needs and goals. They provide individual counseling as needed and conduct support and psych-educational groups on a monthly basis. Our social workers provide participants with resources in the community and referrals.
