The Multi-Disciplinary Team includes all department professionals; physician, nurse, medical social worker, occupational therapist, physical therapist, dietitian, activity coordinator, and program director. Together they determine the medical, psychosocial and functional needs of each participant. Each professional conducts an individual evaluation on the participant and develops a plan of care, including goals, objectives, and services each participant will receive while attending the center.
